Article 29.1.2: Content and form of the request for legal assistance

A request for legal assistance in cases of administrative offences shall be made in writing, signed by the official who sends it, certified by the stamp of the relevant act. and shall contain: (1) the name of the authority from which the request for legal assistance originates; (2) the name and location of the authority to which the request for legal assistance is addressed; (3) the name of the authority from which the request for legal assistance is made; and (3) the name of the authority from which the request for legal assistance is made. Change of administrative offence case and nature of the request for legal assistance; (4) data on persons in respect of whom a request for legal assistance is made, including data on the date and place of the request Name and place of birth, nationality, occupation, place of residence or place of residence and, for legal persons, their place of origin; (5) statement of the circumstances to be clarified and duly indicated 6 information on the actual circumstances of the administrative offence committed, its characterization, text of the relevant act and the present Code and, if necessary, information on the extent of the harm caused by the offence.
